What If I Need My Sample Back?
Samples submitted for clinical testing are retained for two weeks. You can arrange for your sample to be returned.
Requesting Sample to be Returned at Time of Submission
- You will need to submit a written request to have the sample returned.
- Submit the written request at the time of submission, if possible. This can be done by checking the correct box on the requisition form.
- Request must include contact information, destination address and phone number in the request.
- Fees for returns vary by sample, as does shipping. The amount will be calculated and you will be billed.
To request a return after a sample has been submitted, please call us first at 1-866-522-2206 or email CLINICAL@NMSLABS.com to verify availability of the sample and coordinate the shipment destination.
